Grant Athletics


Sports Physicals are required every year to allow for a student/athlete to compete in extracurricular sports. Students MUST be seen by a physician and a completed physical packet turned into Coach Green.  One physical is good for the entire school year and for ALL sports.  

A complete Physical Packet consists of the following:

1) Physical form

2) Concussion form (attached to physical form link)

3) Athletic Code of Conduct 

4) Consent to Treat Form

Elizabeth Green has taken over as the Athletic Director for Grant Middle School Athletics.  Coach Green comes to us with 20 years of teaching experience, 10 years as a middle school athletic director, and 14 years as a head high school volleyball coach.

The Grant Middle School Athletic Department is committed to providing the very best instruction and supervision to its athletes. It is the departments hope that your student-athlete will learn respect for the rules of the game, accept all of the officials' decisions, and always display good sportsmanship regardless of the outcome.

It is also our hope that your student-athlete will have developed the necessary skill set to help him or her achieve his or her goal of participating in high school athletics.

Please be reminded that participation in extracurricular activities is a privilege. Your student must maintain a G.P.A. of at least 2.0 with NO F's and be in good standing with the Grant Middle School administration.
